Our intake process is intentionally designed to prioritize alignment between client and therapist, while connecting you to care in a timely manner. There is no pressure or obligation to work with a member of our team, but if the therapist you request is not available, we may suggest consulting with another staff therapist. Our therapists are eager to work with you! Expect this process to take 2-3 business days, if you monitor your email daily.

To begin the intake process, please fill out the information below or call our intake line at 773-234-3212. After completing this form, our Intake Coordinator will reach out with a set of questions designed to match you with a therapist. If you are using insurance, please have your insurance cards available for verification. Please give us 24 working hours to respond. The intake inbox is monitored Monday - Thursday.

For supervision, mentoring, public speaking, or using our locations to host workshops or other related programming, please email intake@littleseedwellness.com. We will get back to you within 5-7 business days regarding business inquiries.
